Organisations with low injury rates keep having serious incidents, because the measures that look good are counting minor events while the conditions that produce a fatality go unmeasured. This course works on those conditions. Delegates work through process safety as distinct from personal safety, and the major accident hazards a site actually carries. Barrier thinking and the bow tie method follow, including verifying that a barrier is still there. Leading indicators are addressed, since lagging indicators only report what has already happened. Contractor and interface management is covered, given how much high risk work is contracted out. Security is treated alongside safety, covering site access, travel and threat assessment. Environmental obligations, emissions and incident reporting follow. Safety leadership is covered as observable behaviour. The course closes on learning from incidents and near misses without blame, and on assurance that the system works.
